Salary: starting at $16.50- 17.00 /hour. 37.5 hours per week. Opportunity for growth.

Schedule: Monday-Friday, 8:00am-4:00pm. No nights

Location: Sacramento, Arden-Arcade area & Citrus Heights

Adult day programs are person-centered and designed to offer assistance and training in the areas of social, recreational, educational, vocational, and community access to maximize independence.

As an care giver - direct support professional Individual Success Coordinator, you will maintain a caseload of 15 individuals. Duties associated with that responsibility include:

• Work with individuals served and the Planning Team to develop a Person-Centered Plan.
• Responsible for assuring that people served are happy with their services and receiving all provisions as agreed in their Person-Centered Plan.
• Daily documentation/case noting on the progress of individual goals and objectives.
• Assure Annual and Semi-annual reports are submitted to Regional Center for all individuals on the caseload.
• Act as a primary liaison/consultant to clients’ Planning team.

Required to Work:

• CPR/First Aid Certification
• Physical/Back Assessment
• TB Test
• DOJ / FBI Clearance
