The Direct Support Personnel assists with growth and development of interpersonal skills and Activities of Daily Living in the home or community to individuals with MRDD or mental health challenges who live independently (with assistance) in their own residences (supportive living) or a community residential property.Essential Duties and Responsibilities: Provides training/assistance to assigned individuals with Interpersonal skills and Activities of Daily Living in accordance with the outcomes and goals in the individuals support plan and under the direct supervision of the Team Leader, Program Coordinator, or Program DirectorAddresses all outcomes and goals as outlined on the individuals ISP and BSP are performedNo changes in care are initiated without direction from the Team LeaderAny changes in status are reported to Team Leader, Program Coordinator, or Program DirectorDocuments care per company policies and procedures, evidenced by notes are completed, accurate and turned in on a timely basis, and in accordance with documentation policy guidelinesWhen unable to complete an assignment, for any reason, office leadership is informed immediately. Changes in assignment due to consumer or family request must be verified.
