Over 12,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. Job Summary:

Boston Health Care for the Homeless Program (BHCP) is seeking a new Clinical Team Coordinator to lead a group of 7+ care coordinators and/or case managers on our Behavioral Health Community Partners Teams. The Clinical Team Coordinator will be responsible for connecting highly vulnerable and homeless individuals experiencing mental illness and/or substance use disorders with behavioral health, primary care, and social determinants of health supports.
Hours: Full-time; Monday-Friday, 8am-4:30pm
Responsibilities:

  • Oversee outreach, enrollment, and ongoing case management for one of our Behavioral Health Community Partners teams.
  • Identify appropriate services to address social determinants of health, strategically implement a care plan for behaviorally and medically complex patients as well as collaborate and communicate with other care team members; support completion of patient Comprehensive Assessments and Care Plans in a timely manner in accordance with MassHealth requirements.
  • Orient and train staff members to BH CP processes, requirements, and practices; manage team performance on metrics such as hospital discharge follow up.
  • Work closely with care coordinators employed by BHCHP as well as other homeless service organizations; work with senior administration to support quality improvement projects around acute care utilization, quality metrics, disease indicators; support staff trainings.

Qualifications:
To succeed in this role, you’ll bring strong leadership skills, clinical skills, and have the confidence to collaborate within a cross sector team that is based in various locations across Boston, providing feedback regarding population health. You must have experience caring for underserved populations, navigating homeless/housing services and the health care system. Supervisory experience is strongly preferred. In addition, we’re looking for a candidate who will:

  • Manage data reporting and monitor program metrics for the team
  • Provide timely and insightful feedback to care coordinators to help them with their challenging jobs of caring for vulnerable members of our community; provide as-needed staff training on core competencies
  • Lead regular team meetings; direct team flow to meet quality and program expectations Strong problem solving and communication skills (written and oral)

Education and background:

  • At least two years of relevant professional experience and hold one of the following qualifications:
  • licensed registered nurse (RN) with BH expertise; licensed independent clinical social worker (LICSW); licensed mental health counselor (LMHC); licensed marriage and family therapist (LMFT); licensed alcohol and drug counselor I (LADC I); licensed psychologist; licensed certified social worker (LCSW); licensed rehabilitation counselor; or registered occupational therapist.

Compensation and Benefits:

  • The compensation starts at 60,000 annually and increases based on years of experience and license type.

In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars* added to your base hiring rate for demonstrated bilingual proficiency *added to the base rate and the opportunity to work with local hospitals and community health centers.
