The Case Manager reports to the Program Director and is responsible for supporting the daily operations of the Seniors Center, various funding, through conducting case management assessments and service plan compilation and follow up, chart audits, providing monthly case management services, conducting crisis intervention and compilation of reports. This is part time (22.5/hrs) per week
The Case manager must have knowledge of community resources and counseling/social work practices with high risk populations. Case managers must have experience working with persons in crisis while attaining the ability to work independently with a strong sense of focus. Must be task-oriented, nonjudgmental and maintain boundary with individuals. The case manager must be able to work in a variety of settings with culturally-diverse families while having the ability to be culturally sensitive and appropriate. Maintain a caseload and meet with patients on a weekly basis. The goal of the case manager is to help patients regain optimum health or improve functional capability, in the right setting. It involves assessments, comprehensive diagnostic and treatment planning evaluation to achieve wellness and recovery goals. Case managers are the cohort in accessing services via referral, linkage to needed services, and monitoring follow up on patient care in order to meet the individual's needs.
KEY ESSENTIAL FUNCTIONS:
- Provide case management services
- Completes full assessments of participants' history, skills, needs, etc.
- Compile service plans and complete/update service plans as required by regulation
- arrange linkages and referrals to services internally and externally to appropriate services
- Provide on-site crisis intervention
- facilitate individual and group activities with residents
- Facilitate tenant meetings
- Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord/tenant relations, as necessary
- Maintains appropriate documentation of all tenant contacts
- Documents monthly progress notes in the participants' chart
- Conducts home visits to help evaluate participants' compliance with the rules and regulations of the program and to assess the need for referral to supportive services
- Complete monthly reporting requirements that will ensure compliance with the parameters of the program
REQUIREMENTS:
- Fully Vaccinated against Covid-19 required
- Experience with Seniors and Older Adults preferred
- Bachelor Degree Required
- Bilingual (English and Spanish) preferred
- Three years case management experience with experience with substance abuse and mental health homeless population required
