Job Title: Case Manager

Summary/Objective:

La Posada Providencia is an emergency shelter for immigrants in crisis who are seeking legal refuge in the

United States of America. Responsible for advocating for clients, assessing needs, evaluating options, creating plans, and coordinating services.

Essential Functions:

1. Coordinates and facilitates services and care that is safe and effective through assessment, evaluation, planning, implementation, and monitoring.

2. Communicates client needs to several staff members and follows up accordingly.

3. Manage discharge plans upon exiting program.

4. Works collaboratively with clients, families, attorneys, paralegals, social workers, doctors, counselors, etcetera to ensure high quality services.

5. Acts as the client's advocate as it relates to human rights, privacy, and social justice.

6. Maintains the client's comprehensive case management record through detailed documentation.

7. Coordinates with third party service providers (e.g. ProBAR, Catholic Charities, and Refugee Services of

Texas) on a regular basis.

8. Collaborates in the grant and program compliance process by providing information used to populate monthly reports.

9. Helps with the Street Outreach program.

10. Other functions as needed.

Education and Experience:

• Minimum Bachelor’s degree in Social Work or related field
• License preferred
• Human Services Experience
• Management or Supervisory experience
• Experience with disabilities helpful
• Valid Driver’s license
• Bilingual

Supervisory Responsibility:

• Supervises Case Management Aide and Children's Coordinator
• Supervises residents

Additional Eligibility Qualifications:

• Compassionate and able to relate to different clients with various needs.
• Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records.
• Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers.
• Organization to manage several different cases at once.
• Flexibility to change care plans if they are not getting the best results
• Computer literacy to maintain and manage case records.

Job Type: Full-time

Pay: $33,000.00 - $38,000.00 per year
