Your Job as a Case Manager:
The Case Manager is a clinical professional who manages the patients' cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to patients. The Case Manager provides quality care through developing, implementing, managing and evaluating patient care plans.

Full Time hours are early mornings Monday - Friday 5:30 AM - 2:00 PM, allowing you to enjoy a great work/life balance.
Your Responsibilities as a Case Manager:
- Responsible for developing, implementing, managing and evaluating patient care plans;
- Collect clinical data, correlate this clinical data and provide clinical input to the treatment team;
- Accurately convey medical criteria and clinical information between the insurance provider and treatment team as warranted;
- Correlate clinical data for the business office as required.
Your Skills and Qualifications as a Case Manager:
- Bachelor’s degree in Social Work, Counseling or related Human Service field required
- 1-2 years of experience working in behavioral health setting - experience with the addiction recovery population served by the clinic preferred.
- Previous experience in case management, addiction recovery, and/or utilization review preferred.
- Ideal candidate would be bilingual in English/Spanish
