JOB SUMMARY

Case Manager ensures that his/her assigned clients, youth/young adults ages 14 – 24 years old, receive a coordinated range of client-centered services to ensure completion of employment and education programs through the Learn to Earn Program.

  • Provide case management to youth/young adults ages 14 – 24 enrolled in employment and education programs.
  • Conduct comprehensive intake and assessments on clients.
  • Monitors cases by verifying clients’ attendance; observing and evaluating service strategy and responses; advocating for needed services; obtaining additional resources; intervening in crises; providing personal support.
  • Develop and guide youth through a service/plan in accordance with provisions stated by grant.
  • Track and record case information, complete forms and produce monthly attendance and case note reports.
  • Answer inquiries from potential program partners and incoming youth.
  • Develop ways to retain youth (ages 14-24) through motivation and encouragement.

Outreach and network with other youth serving programs and High Schools.

Required Qualifications:

  • Associates or bachelor’s degree required
  • Must have demonstrated case management skills and experience, knowledge of youth development
  • Experience working with inner city youth, human relations, psychology, social work, or related field
  • Excellent interpersonal and written/verbal communication skills and organizational skills
  • Strong knowledge and use of Microsoft and Google platforms

Work Schedule Demands:

  • Full time, 35 hours per week, some evening hours may be required. Due to the nature of this job, this individual must be flexible with their schedule to accommodate the client population

While performing the duties of this job, the employee is regularly required to sit and to talk to hear. The employee frequently is required to use hands, fingers, handle or feel objects, tools or controls and reach with hands and arms. The employee is occasionally required to stand and walk. The employee must occasionally lift and / or move up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The environment is a clinical business office environment. Potential exposure to blood borne pathogens, respiratory diseases and other infectious diseases, and other biohazardous material requiring universal standard precautions. This position requires the individual to have their own transportation, a current unrestricted license to drive, documented auto insurance and the individual must be willing to work in schools, client’s homes and the community.
