Case Manager (Home Health Care) Job at Edison Home Health Care
Edison Home Health Care - is seeking a Case Manager to join its New York City team. The position is full-time and encompasses all five boroughs. Must have at least 6 months of experience working in home health care. Bilingual Spanish – preferred.
Responsibilities:
- Establish relationships with new and existing patients
- Strong knowledge of home care and MLTC
- Strong knowledge of home care MLTC/HMO enrollment
- Strong knowledge of assessment needed to qualify for home care
- Must be able to cover assigned territories throughout the 5 boroughs
- Ability to travel within assigned territory
- Identify clients that are eligible for home care
- Function independently, with personal integrity and ability to work effectively
- Obtain and submit proper paperwork needed for enrollment
- Assist patients with scheduling and coordinating with their community case manager for State and MLTC assessments
- Assist patients with their state and MLTC assessment
Requirements:
- Must have a minimum of 6 months home care experience.
- Must have vehicle and valid driver’s license
- Technology savvy
