Job Description:

  • Assess the need for Medicaid, Public Assistance, HRA 2010e supportive housing application and complete all necessary documents to facilitate this process in coordination with a Social Worker, including follow-up to receipt of eligible services.
  • Ensure completion of all Brad H related tasks/mandates for selected patients in the assigned facility.
  • Enter client information into the H+H database.
  • Forward the appropriate documents to the Department of Homeless Services for all class members that are un-domiciled.
  • Communicate with SPAN, LINK, Community Based Organizations and concerned family members in an effort to exchange information that would assist with the transition of the class member to the community.
  • Assess and issue the Medication Grant Program (MGP) card and ensuring continuity of psychotropic medication and after care letters upon release.
  • Collaborate with other Mental Health Service team members to facilitate the delivery of services to the patient population through a mission-driven process that views services globally from incarceration to the community.

Participate in therapy rounds, team meetings, unit activities; court/diversion advocacy and group interventions.

  • Coordinate care for patients on unit with preparation for discharge from jail environment to community or other facility.

Preferred Skills
Experience working with individuals with severe and persistent mental illness. Knowledge of benefit entitlements, community mental health and substance resources for the mental ill patient population.
Minimum Qualifications
1. Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs; or,
2. Education and/or experience equivalent to (1 above). Study at an accredited college in Sociology, Psychology or other behavioral sciences may be substituted on a year-for-year basis for the experience described in (1 above). However, all persons must have at least one year of full-time experience as described in (1 above).
