The Five Rivers Health Center is an expanding Federal Qualified Health Center located in Dayton and Xenia, Ohio providing a medical home for low-income patients. Position Overview
The Case Manager must abide by various regulatory compliance rules and must always treat patients with dignity and respect. The Case Manager will assist the Community Health Workers and participants in securing appropriate employment within the community.
General Responsibilities
- Assess the possible needs of patients by interviewing patients, reviewing medical records, and consultation with primary care provider.
- Assist participants in contacting and obtaining interviews from prospective employers
- Assists with power of attorney, living will, and utility paperwork.
- Determines appropriate resources in the community to help overcome patient’s barriers.
- Providing guidance to the Community Health Workers
- Other duties as assigned
- Local travel is required
Qualifications
- High School diploma or equivalent required
- Associates Degree in the healthcare or business field preferred
- 3+ years of Community Health Worker experience required
- Windows-based computer experience required
- Must have valid driver’s license and dependable transportation
