Utah Valley Pediatrics is looking for a Full Time Care Coordinator in our Payson Office!

General Summary:

The Care Coordinator will assist physicians and staff with clinical and clerical functions for patients needing coordination of health care needs. Work closely with physicians, staff and parents to improve the health and satisfaction of patients with chronic and complex diagnosis. Coordinate access to appropriate health services. Educate patients and families to help manage their health care needs. Functioning with a high degree of autonomy, communication, and interpersonal skills is a must; as well as understanding the health care system and be able to solve complex problems.


Qualifications:

Meet One of the following criteria:

  • Registered Nurse
  • Experienced Medical Assistant
  • Bachelor's degree in health care-related field with clinical experience


Additionally:


  • Five years of clinical experience
  • Exceptional communication skills
  • Skills in assessment, critical thinking and problem solving
  • Care management experience


Essential Job Responsibilities:


  • Collaborate with PCP and other staff members to coordinate care of patients with complex diagnosis.
  • Proactively contact PCP regarding patients with complex needs.
  • Maintain expert knowledge of complex patient conditions including psycho/social needs.
  • Maintain expert knowledge of internal and external resources.
  • Assist physicians in preparing medical care plans with patients and monitor patient compliance with care plans.
  • Refer specialty services as appropriate and with PCP consent.
  • Facilitate patient access to appropriate services, i.e.: specialist, community resources, parent advocates.

Schedule:

This position will work typical full time hours, final schedule will be determined by doctors and office manager

Responsibilities:


  • Demonstrate professional, appropriate written, verbal and nonverbal communication with patient, families, medical staff, colleagues, and other physicians to promote continuity of care and services to enhance clinic goals for health care.
  • Build positive relationships; advocate for patient and their families.
  • Maintain patient confidentiality at all times, and follow HIPAA guidelines and regulations.
  • Proactively contacts patients to arrange follow-up care for special health care needs.
  • Types correspondence and is able to maintain complete patient records.
  • Demonstrates organizational skills.
  • Understand and use current information systems.
  • Recommend changes and strategies needed to improve clinic programs.
  • Maintain registry of patients with special health care needs proactively help patients on registry to acquire needs for each patient.
  • Conduct self in accordance with Utah Valley Pediatrics Employee Handbook.
