Summary
Identifies, develops and maintains lead sources for Assisted and Independent Living. Manages electronic referral programs, monitors turn-around time, and conversion ratios. Converts referrals to tours and admissions. Working knowledge of payer sources. Facilitates all aspects of a smooth transfer/admission process. Identifies and develops referral accounts for the generation of referrals, with monthly/quarterly plan to maintain facility budgeted census. Conducts annual Market Analysis; or other frequency as census dictates.
Qualifications
Education:
- Graduate of high school or GED. BS degree preferred.
Experience:
- A minimum of two years admission experience, of which one year in a long-term care environment.
- Experience in either an administrative or consultant role, or involvement in a survey process.
- Customer services, sales and/or marketing experience preferred.
Essential Functions:
- Receives leads from providers, facilities, or any customer source, and converts appropriate leads to drive census.
- Manages and coordinates leads from key hospitals in the market.
- Assist in interviewing residents/guardians/sponsors and obtaining required admission information and signatures and maintaining/training backup team while external.
- Provide residents with admission information packet (e.g., resident rights, notice of privacy practices, admissions contract, etc.).
- Obtain the resident/responsible party’s signature on all required permits, releases, authorizations and admission documents, or verify that backup team member has completed.
- Review/explain the resident, and/or guardian, room rates, billing procedures, visitors/guest privileges, restrictions, resident care procedures, etc., as appropriate.
- Collect, assemble, and check admission papers. Forward to appropriate department.
- Maintain an accurate record of available beds.
- Coordinates/communicates closely with facility Team.
- Distribute information regarding admissions to appropriate departments; i.e, including nursing, business office, attending physician, and other departments as identified by the Inter-disciplinary Team, or follow-up to assure backup team member has efficiently.
- Agree not to disclose assigned user ID code and password for assessing resident/facility information and promptly report suspected or known violations of such disclosure to the CEO.
- Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the CEO.
- Obtains the necessary medical and financial information to qualify patients for admission.
- Assist the MDS Coordinator in scheduling resident assessments or delegate to Social Services as facility size dictates.
- Facilitates patient placement based on clinical services needed, bed availability, patient/family preference, geographic location, etc.
- Develop and maintain facility Marketing Plan with Inter-disciplinary Team.
- Conduct Annual Market Analysis; or more frequently as census demands.
- Perform miscellaneous duties and assist Inter-disciplinary team as directed or required.
- Maintain clean and efficient work area as attractive place to meet with potential admissions.
- As necessary, assists with community, hospital, physician and other referral source events.
Knowledge/Skills/Abilities:
- Skilled in clinical and financial review of referrals.
- Ability and willingness to travel frequently (at least 80%) to obtain referral information and/or facilitate an on-site admission.
- Ability to work flexible hours.
- Ability to work independently with minimum daily direction, and to self-motivate.
- Ability to communicate effectively with patients and their families, physicians, hospital administrators, and at all levels of the organization.
- Knowledge of, or ability to learn, reimbursement programs.
- Ability to conduct oneself in a mature and professional manner.
- Meets all health regulations, and passes post-employment physical examination, if required.
- Ability to maintain confidentiality.
