PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Interim Housing team as the Outreach Case Manager at the Venice location.
ABOUT OUR TEAM
Our Street Outreach programs to work identify and build relationships with individuals currently experiencing homelessness and living on the streets. These programs utilize PATH’s Street Outreach Integrated Care approach to assist clients in breaking the cycle of homelessness by providing field-based interventions, case management and supportive services.
WHAT IS AN OUTREACH CASE MANAGER
Outreach Case Managers play a key role at PATH as one of the first points of contact for homeless clients in the Venice community. They utilize PATH’s Street outreach integrated approach to provide both outreach and engagement and case management interventions to quickly assess and link clients who are currently homeless and living on the streets with supportive services. Position Responsibilities include:
- Conduct daily street outreach to develop relationships and build rapport with clients in order to assist them with finding a home.
- Conduct VI-SPDAT, (Vulnerability Index- Service Prioritization Decisions Assistance Tool), on all new clients in order to connect them to proper intervention and services. Document results, complete match initiation form, and submit all paperwork to CES Coordinator.
- Meet with established clients on a weekly basis to review, evaluate, and support goal completion.
- Provide street-based, mobile case management services designed to connect homeless clients to long-term supportive services
WHAT YOU BRING
We’re looking for candidates that:
- Demonstrate knowledge and experience with evidenced based interventions
- Engagement and rapport building with hard-to-reach clients.
- Harm Reduction and Motivational Interviewing strategies.
- Housing First.
- Non-violent crisis intervention techniques.
- Case management interventions that have been proven effective in vulnerable populations.
- Ability to work independently and within a team environment and exercise mature judgment.
- Excellent interpersonal skills, ability to work in a variety of settings with culturally diverse persons and communities with the ability to be culturally sensitive and appropriate.
- Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations
PREFERRED QUALIFICATIONS
- Two (2) years working in homeless services, mental health, with vulnerable populations providing supportive services, or other relevant fields.
MINIMUM QUALIFICATIONS
- All levels of education and experience welcome.
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire: Tuberculosis Test

- Have reliable transportation
- A valid driver’s license

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.

PATH is a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
