What You’ll Do:

The Wraparound Facilitator provides direct support and education to the family, all Child and Family (CFT) members and the team assuring fidelity to the Wraparound process model. This service is provided through scheduled treatment sessions and may be provided to individuals, families, or groups.

Essential Job Functions:

  • Bachelor's Degree (human services field preferred) from an accredited institution
  • Recent College Graduates interested in pursuing Graduate Level Programs in Social Work/Psychology/OT/PT or Nursing encouraged to apply
  • Direct support experience with both developmental disabilities and mental health populations preferred
  • Experience with crisis prevention and intervention services preferred
  • Maintain valid license and insurance for personal vehicles utilized for work
    • Please note this position requires Weekend availability. The salary for this position is: Between $14-$16/hour depending on education and experience.
  • Complete within specified time period the trainings as required by agency and service model

Environment:

  • Work is office/community-based; travel and work within any setting is required. Exposure to weather elements may occur during travel.

What We’re Looking For:

To join our team, you must be willing to provide care in a crisis group home and mobile setting, have a passion for helping others, have experience with MS Office suite and data entry, plus strong verbal and written communication skills. We also require the following:

  • Bachelors Degree in Social Work, Psychology or related Human Services Field
  • Direct experience with High Fidelity Wraparound preferred
  • Valid unrestricted, state appropriate driver’s license and ability to be insured by Easter Seals UCP’s insurance carrier
  • Must have reliable transportation and current vehicle insurance

Essential physical requirements:

  • Must physically be able to lift a minimum of 50 pounds, bend, and assist individuals with mobility needs
  • Must be able to hear within normal ranges
  • Must be able to physically engage in recreational activities (basketball, walking, tennis, etc.) with individuals on a level and for a length of time that is therapeutic for the individual
  • Must be able to walk in order to articulate crisis prevention, intervention, and de-escalation supports
  • Must be able to see with or without accommodation to observe individuals
  • Must be able to operate a standard automatic passenger van
