• Acti-Kare Responsive In-Health Care is seeking a Team leader to fill the position of* In-HomeCare Manager * for our office. The Manager leads field staff on client service, clinical excellence, quality, and adherence to policies and procedures. The Manager is also accountable for internal case management and field staff evaluation and development.

*Primary Responsibilities:*
Demonstrate and communicate the core values of Actikare Responsive In-Home Care policy. Work closely with the Retirement homes and Director to ensure cohesive and consistent management of all field staff. Oversee the delivery of all services to ensure superior quality. Review all clinical documentation to ensure consistent, accurate, high-quality services. Work with other office team members to ensure that the most appropriate clinicians are assigned to each client. Ensure development, implementation and updates to the client s individualized plan of care based on continual assessment of the client. Ensure that all documentation progresses. Coordinate client care, referrals, and act as internal case manager and client advocate. Participate in the hiring of field staff and interview process, as applicable. Complete regular field staff performance evaluations with Client Services Manager s input. *Qualifications:*

* Graduate of an accredited and Certification.
* Prior supervisory experience commensurate with program requirements.
* Minimum of two (2) years of community homecare experience.
* Demonstrated record of strong interpersonal skills.
Smartcare experience is desirable as full training will be given. Knowledge of Homecare is essential.
