GENERAL SUMMARYWorks under the supervision of the Manager of Social Services. The SW Student Discharge Planner Assistant's primary job function is to provide assistance to patients and their families in the form of discharge planning and resource referral appropriate to the ages of the patients served. Performs other duties as assigned.

QUALIFICATIONS

Education:
-
High School Diploma or successful completion of an equivalent High School Exam Required

Enrolled in Bachelor's Degree program required.

Licensure:
-
BLS within 90 days of hire required.

Experience:
-
Participation in a related college practicum preferred.
-

Interpersonal Skills:
-
Substantial interpersonal skills necessary to obtain information from patients and families.
-
Provides counseling to the same and interact with patient, family, and hospital patient care personnel.

Essential Technical/Motor Skills:
-
Ability to input information into the computer.
-

Essential Physical Requirements:
-
Physical demand level is sedentary to light.
-
Ability to use hand for repetitive simple grasping, fine manipulation and pushing/pulling when writing and using other office equipment.

Stands and/or walks for one to four hours per day when visiting patients in their rooms.

Sits for five to eight hours a day when formulating treatment plans for patient and counseling.

Essential Mental Requirements:

Ability to read and interpret data and to compile statistical reports.

Ability to assess and evaluate patients.
-
Pays close attention to detail when interviewing patients, providing counseling, and preparing discharge plans, etc.

Essential Sensory Requirements:

Ability to hear when obtaining necessary information from patients and speak clearly when counseling.

Occasionally uses good vision when completing treatment plans and other reports.

Exposure to Hazards:

Normal office equipment or patient care environment.

Occasional exposed to odors, body fluids, and infectious diseases.

JOB SPECIFIC DUTIES AND PERFORMANCE EXPECTATIONS
- Arranges continuing care post discharge including coordination of services with community resources.
- Responds to high-risk screenings and physicians' consults according to criteria.
- Maintains accurate record keeping in patient's chart and departmental statistics.
- Participates in team meetings, non-departmental meetings and on-going hospital committees.
- Maintains skills through continuing education and current knowledge of community resources.
- Arranges discharge planning needs by meeting and speaking with patients and families in addition to collaborating with providers, nursing, therapy and other skilled disciplines.
- Serves as a liaison between patients, families, physicians and advanced care practitioners, organizations, and outside agencies.
