The City of Fishers is seeking a full-time Social Worker - Health Outcomes & Crisis Intervention Specialist to join the Fishers Health Department.
Under the supervision of the Assistant Director of Public Health, the incumbent will be responsible for providing leadership in program development and administrative oversite to preventative health and behavioral health efforts in the City of Fishers. Crisis intervention including provision of short-term clinical therapy, resource connection, and case management services are anticipated to be part of this role. Experience collaborating with a variety of community agency representatives, as well as extensive knowledge of services available in Hamilton County will be needed. This individual will work closely with public safety, including community paramedicine and police to accept referrals and assure follow up on individuals identified with health needs of various types, including behavioral health, through those channels. Coordination and collaboration with area hospitals, schools and other agencies is a critical component of the role. The primary objective of this position is to support the extension of efforts started through the community paramedicine program, and to extend those efforts to a more community-focused, public health-based preventative approach to physical and behavioral health issues.
This is currently a 2-year grant funded position.
Examples of Duties
- Provides short-term clinical therapy services as needed while connecting to long-term supports.
- Conducts group sessions and trainings as indicated.
- Seeks out and obtains additional trainings and certifications based on needs of community and program goals.
- Coordinates referral services and follow-up care with community agencies, schools, support groups, medical providers, and others to assure resources for long term success.
- Conducts services in home, office or clinic space as determined by needs of the resident.
- Provides expertise to the department on matters pertaining to clinical mental health needs of the community.
- Assists in developing and implementing community-based, preventative health and behavioral health programming based on community needs and gaps.
- Assists in ongoing quality assurance and data review efforts of the cross-departmental initiative to assure efficacy.
- Develops and maintains accurate records in compliance with applicable guidelines and rules
- Successfully advocates on behalf of at-risk populations to assure their concerns are addressed.
- Works collaboratively with all team members to assure an equity-focused approach to public health outreach services.
- Attends events and collaborates with team members to conduct community outreach as needed
- Maintains working knowledge of the Fishers community, neighborhoods, and organizations, and refers residents appropriately based on need.
- Other duties as assigned.
Minimum Qualifications
- Master’s degree in social work, counseling, or related field from an accredited program;
- At least 2 years of professional work experience;
- Active licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) is preferred. Consideration will be given to other certifications such as LCAC, LMFT and combination of education and experience;
- Valid Indiana Driver’s license required;
- Community, non-profit, public health, or health advocacy experience highly preferred;
- Attention to details required;
- Must exhibit flexibility working with a wide variety of populations and organizations;
- Demonstrated competency with professional software including Microsoft Office applications, medical records systems, and proficiency with learning new IT software;
- This role will require occasional evening and weekend hours based on business needs.
