Envision Healthcare at Home is looking for PRN (Per Diem) Hospice On-Call Registered Nurses who are passionate about providing quality care to patients and families enrolled in our Hospice programs within Denver metro area.

The RN on-call position provides support for home health patients and their caregivers after hours. On-call shifts begin at 5:00 PM through 8:30 AM Monday through Thursday, and from Friday at 5:00 PM through Monday at 8:30 AM. PRN is as needed within these hours.

Responsibilities include:

• Client assessment
• Answering incoming after hours phone calls from patients or caregivers. Answers questions, provide support and education as appropriate.
• Makes visits to patients as needed/requested to respond to urgent needs or significant changes in patient condition
• Admits new patients
• Fulfilling patient care plans
• Intervention and overall care of patient care
• Following physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
  
• Strong commitment to delivering exceptional care while demonstrating compassion and integrity
• Comprehensive and precise communication skills
• Provide one-to-one care with your patients in their homes, assisted living facilities, long term care

Qualifications

• Graduate of an accredited or state approved school of practical nursing
• Current driver's license, auto insurance and reliable transportation
• Maintain a current CPR certification
• Licensure as a LPN in the State of Colorado
• A minimum of one year of experience in an acute care setting; home care/hospice experience preferred.
• Must be willing and/or able to comply with all state/federal COVID mandates.
  
• Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist
• Ability to work independently
