We are seeking a qualified and motivated full-time RN, Memory Care Coordinator
Responsibilities:
- Coordinates internal and external resources to meet identified needs.
- Monitors and evaluates effectiveness of the care management plan and modifies as necessary.
- Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
- Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
- Work closely with the Director of Nursing and LPNs to coordinate best practices and procedures.
- Participate in the on-call rotation for evenings and weekends.
Qualifications
NJ RN Licensure. Minimum 1 year experience preferred.
Requirements:
- Valid RN license in State of N.J.
- BSN preferred
- Nursing experience in a clinical setting
- Must be computer literate and experience with EMR preferred
- Effective writing and oral communication skills are required as well as proven tasks records with team building
