Visiting Angels - West Metro is looking for a compassionate, dedicated RN Case Manager to join our team. Home Care is one of the fastest growing areas in the healthcare industry. The RN Case Manager coordinates the clients’ home care services and follows physicians’ orders in accordance with agency, state and federal guidelines and is responsible for supervising the caregiver staff. This position reports to the Director of Nursing.

• West & Southwest Metro - Twin Cities
• Flexible Schedule, day shifts. Full-time. No weekends or holidays.
• Pay Range: $72,000-90,000 DOQ. Benefits eligible.
• PTO plus 6 paid holidays

Responsibilities:

• Supervise caregiver staff. Recognize and reward good job performance and promote caregiver recognition.
• Conduct caregiver orientation and regular training per agency guidelines and regulatory standards.
• Conduct in-home supervisory visits in accordance with agency guidelines and state licensure regulations.
• Conduct client assessments to determine Plan of Care and finalize service agreements. Assure that clients and their families understand the Plan of Care and their responsibilities.
• Initiate the Plan of Care and make necessary ongoing revisions.
• Respond to and resolve client complaints or concerns regarding care.
• Maintain a safe environment for clients.
• Maintain absolute confidentiality of all information pertaining to clients and clients’ family members.

Requirements:

• Currently Registered/Licensed with the State of Minnesota Board of Nursing with good standing.
• 3 years of previous experience in home care industry preferred.
• Must have a valid driver’s license and reliable transportation.
• Successful leadership background with the ability to mentor others.
• Ability to work independently and efficiently manage time with minimal supervision.
• Excellent communication skills with an understanding of aging in place.
• Ability to lift 50 pounds.
• Possess and maintain current TB testing.
