Our Dover Center for Health and Rehabilitation is looking for an RN Case Manager / Discharge Planner to join their growing facility! Experience with discharge planning preferred.

What You Will Do as am RN Case Manager/ Discharge Planner:
- Develops, implements, and reviews healthcare plans for patients that are geriatric, recovering from serious injuries, or dealing with chronic illnesses.
What You Will Need as an RN Case Manager / Discharge Planner:
- RN license in New Hampshire (or the ability to obtain one)
- Previous Experience within Case Management
