GENERAL SUMMARY:


Provides nursing services on an intermittent basis to homebound patients in their places of residence in accordance with an established Plan of Care. The Scheduler makes patient assignments.


PRINCIPAL DUTIES AND RESPONSIBILITIES:


1. Initial evaluation and assessment of patient needs.


2. Establishes a nursing care plan, implements this plan, and evaluates patients’ progress. Sets goals with patient, patient's family and physician.


3. Maintain safe environment for patient.


4. At regular intervals, reports patient condition to physician.


5. Completes verbal orders within 24 hours and maintains an up-to-date medical record, being certain that the

information is accurate, pertinent, current and meets all Medicare and third party reimbursement criteria.


6. Provides services requiring substantial specialized nursing skill.


7. Communicates, on a regular basis, with other disciplines involved in the care of the patient.


8. Promotes optimum health levels for patients.


9. Attends regularly scheduled staff meetings.


10. Complies with Agency stated policies and procedures.


11. Accepts accountability for his/her actions and direct responsibility for his/her patients.


12. Adheres to the Agency Infection Control guidelines.


13. Documents patient care in a manner that is both outcome directed and addresses each individual intervention

included in the patient plan of care.


14. Insure that documentation indicates that interventions, outcomes, and goals included in the plan of care have

either been met or modified. If modified, documentation must reflect reasons.


15. Complete all discharge summaries within 24 hours of discharge and/or "discharge for service".


16. Insure that all discharge summaries will reflect care provided, outcomes and goals met or abandoned.


17. Complete all visit documentation within 24 hours from the time service is provided.


18. May have access to electronic or manual protected health Information by scope and responsibility. Will be

required to adhere to the privacy practices as detailed in the Notice of Privacy Practices, privacy policies and procedures.


19. Maintains Professional License by completing the State mandated Child Abuse CEU’s.


20. Assumes other duties and responsibilities as assigned by management.


OTHER DUTIES AND RESPONSIBILITIES
:


1. Maintains confidentiality of patient information.

2. Supervises ancillary personnel and delegates responsibility.

3. Home Health Aide evaluations are completed and recorded at least every 10-14 days.

4. Adheres to Agency dress code.

5. Maintains up-to-date visit calendar.

6. Participates in planned in-service programs and mandatory in-services once per year.

7. Attends meetings.

Health Care Services.

19. Ability to perform duties within the standards of practice of the Professional Nurse Law.


KNOWLEDGE, EXPERIENCE, LICENSURE REQUIRED:


1. Graduate of an accredited School of Nursing.

2. Maintains current Pennsylvania Nursing License.

3. Possesses two (2) years of current nursing experience.

4. Valid current driver's license and necessary insurance coverage as mandated by the state.

5. Specialized Nursing knowledge, preparation, experience, skin and competency to perform required or assigned

duties.

6. Maintains current CPR certification.

PHYSICAL JOB DEMANDS:


1. Ability to physically move, lift, shift 100 pounds.

2. Ability to stand, walk, climb and descend stairs, reach above objects involved in the care of a patient.

3. Able to safely operate a motor vehicle.


WORKING CONDITIONS:


  • Normal working hours are Monday through Friday, 8:00 a.m. to 4:30 p.m. and one weekend per month or as

demand to meet patient needs. Nurses can always communicate with the manager on call 24 hours a day.

2. Cell phones are carried by all RN's and are to be operational between the hours of 8:00 a.m. to 4:30 p.m.

3. Nurses who see weekend and holiday patients can communicate with the manager on-call 24 hours a day.

4. Maintain a safe driving record and a vehicle in dependable working order.
