Compensation (Pay): $65,927.17 to $95,594.40/yearly, based on experience.
A $5000 sign on bonus will be paid in the following manner: $1250 payout upon 30 days of employment, $1250 payout upon 90 days of employment, $1250 payout upon 6 months of employment, and $1250 payout after 9 months of employment*.

Education/Experience:
Education: Graduate of an accredited Nursing Program required. Bachelor of Science in Nursing preferred.
Work Experience: Minimum 1-3 years of nursing experience required with a minimum of 1 year in nursing leadership preferred; ambulatory care/primary care setting experience preferred; knowledge of process improvement or infection control and/or Risk Management in a healthcare setting preferred.
Computer Skills: To perform this job successfully, an individual should proficient with MS Office application and databases.
Certificates and Licenses: Current Colorado RN License required. Colorado Driver's License and Insurance as required by state law.

Job Summary: Supports Peak Vista Community Health Center's (PVCHC) Risk Management, Compliance and Quality Assurance (QA) programs through the management of potential risks and liabilities for the organization and ensures compliance with all regulatory requirements.
Essential Duties and Responsibilities:
- Act as a clinical and administrative liaison to identify, evaluate, and reduce the risk of injury to patients, staff, and visitors and to reduce the risk of loss to the organization (loss may be described in terms of employee numbers, finances, organization's reputation, etc.).
- Ensures Regulatory and Accreditation requirements are met to include Health Resources Services Administration (HRSA), Federal Tort Claims Act (FTCA), Occupational Safety and Health Administration (OSHA), etc.), researches and recommends appropriate educational offerings for all providers and staff based on specialty and high-risk determinations for HRSA and completes the Risk Management portion of the annual HRSA Deeming Application in conjunction with Compliance and QA.
- Develops and implements an annual Risk Management Plan
- Completes quarterly and annual “Risk Management” reports for PVCHC's Board of Directors and ensures HRSA reporting requirements are met.
- Develops and implements the annual Risk Management Training Plan for all staff based on identified areas/activities of highest clinical and other risk for PVCHC.
- Provides Risk Management training for PVCHC's Board members and staff (via a Skills Fair, HealthStream, or other appropriate method) annually and as needed, based on trends and risks identified.
- Evaluates, investigates, and directs the resolution of applicable incident reports by overseeing incident tracking and reporting.
- Identifies, assesses, and mitigates organization risk using formal and informal mechanisms for risk identification such as incident reporting processes, root cause analysis, risk assessments, audits, review of patient complaints, analysis of trends, claims analysis and review of pertinent quality improvement information with a goal of improved patient safety and protection of the organization assets.
- Collaborates with clinical staff to ensure timely risk management review and management of serious patient events and medical errors to improve quality and safe delivery of patient care.
- Completes site visits with nursing as indicated to assess Health Centers for risk management concerns as needed.
- Reviews and revises applicable Policies and Procedures (P&P) on an annual or “as due” basis. P&Ps the Risk Manager is responsible for include “Risk Management Plan,” “Incident Reporting,” “Patient Grievance Process,” “Patient Behavior and Dismissal Process”. The Risk Manager will be responsible to review and assist with revision on other appropriate policies annually as directed by Compliance.
- Provides early investigation and intervention for adverse occurrences.
- Guides investigations, education, and actions to take with all medical errors.
- Monitors and responds to the Patient Grievance phone line; provides investigation and resolution in real time when possible. Provides guidance and coordinates efforts with department and/or Medical Directors, Vice Presidents, Human Resources, and other resource teams as necessary.
- Provides guidance for investigation, actions, and the proper process steps and documentation for patient behaviors and coordination of patient discharge from care when appropriate.
- Serves as the primary expert for the incident reporting management system and provides training and support to staff as needed
- Completes and presents a bi-monthly “Medication, Immunization, and Prescribing and Dispensing Error” report for the Pharmacy and Therapeutics Committee.
- Works with Employee Health, Compliance, Safety and Security, and PVCHC's Worker's Compensation insurance carrier to meet accreditation standards and to keep the Experience Modification Rate (eMod factor) as low as possible.
- Participates in appropriate committee meetings (Workers' Compensation Quarterly Report with the WC Insurance Team, Pharmacy and Therapeutics Committee, and others as assigned).
- Makes recommendations concerning protocols and processes throughout the organization as these relate to risk prevention and mitigation.
- Attends Risk Management educational offerings from Emergency Care Research Institute (ECRI) and other appropriate sources to ensure knowledge in the field is up to date.
