Adapt Integrated Health Care is currently recruiting for a Full-Time Resource Navigator to join the Grants Pass team!

Position Purpose:

This is a contracted service that will provide coordination, referrals, barrier eliminations for DHS/CWP clients for housing and related issues.

Essential Job Responsibilities:

  • Assist client in securing and maintaining safe and sober housing by identifying client needs, providing referrals to meet those needs, and removing barriers.
  • Assist Client in building support systems outside of DHS and outside of alcohol and drug treatment.
  • Transport DHS/CWP clients to initial treatment services (A&D, MH, or physical health) if needed and other meetings.
  • Assist client in finding resources for safe and sober housing and child care to support sober living.
  • Provide CAF caseworker with written and electronic documentations of all client activity.
  • Attend court hearings, treatment staffing, and other meetings.
  • Record all client activity for monthly data reports and tracking of cases.
  • Identify barriers to safe and sober housing and work to help the client overcome them.
  • Other duties and tasks assigned by supervisor.

Knowledge/Skills/Education Required:

Bachelor's Degree or Associates Degree and 1 year of work experience in social services.

Additionally, required to have a driver's license with MVR that qualifies for agency insurance coverage. Qualifications to meet requirements as a Peer Delivered Services Provider preferable.

Pay: $15.00 - $17.00 per hour
