Position Summary:

The Admissions RN is responsible for assessing patients and families who request admission to Montgomery Hospice. During assigned visits, the Admissions RN provides a comprehensive assessment of patients to be admitted and initiates the plan of care and functions in other nursing capacities as needed.

Minimum Qualifications:

• Current state of Maryland Nursing License. CHPN preferred.
• Graduation from an accredited nursing program. B.S.N preferred.
• Two years of recent medical-surgical experience, preferably with oncology/multi-system failure.
• Previous hospice or home experience strongly preferred.

Duties and Responsibilities:

• Accurately assesses patients and family needs.
• Identifies and recommends medical equipment and supplies that will assist in maintaining patient’s comfort and caregivers’ ability to provide care.
• Identifies the need for additional services, including nutritional assessment, physical therapy, occupational therapy and speech therapy and initiates appropriate referrals.
• Assesses need for and educates caregivers on safety and emergency preparedness, including safe use of medical equipment.
• Conveys physician’s requests to appropriate staff on appropriate forms.
