Registered Dietitian Job at Optimal Home Care and Hospice
Assists and counsels clients and families with nutritional needs.
- Master's degree from a school of nutrition accredited by the American Dietetic Association and registered with the state or meets all education and experience requirements for licensure.
- Minimum of 1 year experience in dietetics preferably in home health or a hospital, outpatient clinic, rehabilitation center, or mental health program.
- Demonstrated knowledge and skills necessary to provide care primarily to the adult and geriatric population
- Demonstrated knowledge of the principles of growth and development over the life span
- Able to assess data reflecting the client's status and interpret the appropriate information needed to identify clients' requirements relative to their age-specific needs
- Demonstrates computer skills sufficient to use the electronic medical record effectively.

Job Duties/Knowledge
- Provides Registered Dietitian services in accordance with physician's plan of treatment
- Assists the physician and other team members of the home health team to understand the significance of nutrition factors related to the client's care
- Assesses and reassesses nutritional care plan in order to help the client and family cope with problems in meal planning and food choices and follow medical recommendations to maximize the client's health condition
- Counsels clients and families concerning the facilitation of the medical plan of treatment
- Recognizes and utilizes community and family resources to assist the client in long-term nutrition care or to learn to live within the client's disability and/or illness
- Develops, prepares, and maintains individualized client care progress records with accuracy, timeliness, and according to policies.
- Keeps abreast of nutritional trends and knowledge
- Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager
