Job Summary:

The Referral Coordinator processes and facilitates Health Center patient referrals for medical specialty services not available by the Health Center. This position ensures specialty referrals are processed accurately in a timely manner, including completion of insurance referrals and authorizations, documentation and communication to the patient, specialty provider and ordering provider. This position ensures that specialty appointments were attended, results received and communication to Primary Care Provider.

Essential Functions:

  • Process and facilitate specialty referrals for patients of the Health Center.
  • Communicate with managed care plans and third party payers to obtain approvals for patients to be seen by the specialists.
  • Communicate to patients to confirm the appointment time, location, provider name and practice information and educate the patient on what material or documentation to bring to the appointment.
  • Collaborate with EMKCHC specialty referral network to ensure specialty providers are familiar with Health Center referral practices and the requirements of managed care plans with which the Health Center contracts.
  • Track consultant reports and diagnostic results to ensure patient’s attended their appointments and results were received. Communicate with Primary Care Providers for missed appointments.
  • Manage telemedicine referrals, including sending patient data sheet, uploading consults into the patient health record, communicating with Primary Care Provider, booking patient appointment and communicating appointment information to patient.
  • Manage and respond to patient portal messages.
  • Produce reports and maintain data systems as requested including outstanding referral reports and referral follow up reports.
  • Performs other duties as assigned.

Education and Experience:

  • Minimum High School Diploma or equivalent.
  • Excellent organizational, time management skills.
  • Moderate experience with spreadsheets and databases demonstrated with basic-advanced level computer training certificates and/or 1-2 years database/spreadsheet experience.
  • Demonstrate an awareness of what needs to be done and plans and organizes work for greatest efficiency; shows self-starting motivation to begin and complete work without prompting.
  • Ability to prioritize job related activities in order to take action when needed.
  • Minimum 1 year healthcare experience working with scheduling appointments OR 2 years customer service experience.
  • Bilingual in English /Spanish or English/Portuguese preferred.
