JOB DESCRIPTION
JOB TITLE: Qualified Intellectual Disabilities Professional (QIDP)
JOB FUNCTION: Ensure that implementation strategies are developed outlining services and supports that will assist individuals to pursue the outcomes outlined in their Personal Plan utilizing the Interdisciplinary Team approach. Provide linkage with agency, community, and state components

JOB RESPONSIBILITIES
Actively support and further the Mission of The Arc of the Quad Cities Area. " To empower people with disabilities to believe in their own unique abilities and achieve their full potential by providing quality, innovative services that focus on advocacy, independence, employment, meaningful community life and personal happiness."- Develop implementation strategies for assigned individuals through the Interdisciplinary Team approach. Provide linkage to services within the agency as outlined in implementation strategies for each assigned individual on his/her caseload.
- Ensure compliance with federal rules and regulations, DHS, Medicaid, and CARF guidelines insofar as they pertain to service delivery.
- Monitor all services provided to the individual and to ensure proper implementation of the person-centered plan.
- Complete ICAP and coordinate/schedule individual reviews and assessments as needed.
- Complete all individual/program documentation, progress notes and other reports as required.
- Complete discharge summaries and all follow up when an individual passes away. Ensure report is thorough and complete.
- Serve as the main contact person for parents, family and any other interested individuals agencies for each individual on his/her caseload.
- Advocate for each individual on an on-going basis.
- Provide back-up support for assigned CILA home(s) on an as needed basis.
- Ensure all clinical records and files are maintained for each individual in compliance as required by regulatory agencies as well as agency policy and procedures.
- Be willing to provide 24-hour on-call coverage for Residential Alternatives by carrying a cell phone if assigned. Must live within cell phone range. In the future, the position may supervise a CILA home.
- Attend meetings, training sessions and perform other duties as assigned.
QUALIFICATIONS
Education: BA in Human Services from an accredited college or QIDP qualifications.
Driving: Valid driver's license in good standing and appropriate auto insurance required. Must be approved by Agency's Auto insurance carrier for driving agency vehicles.
Strength: Ability to lift minimum of 50 lbs.
QUALIFICATIONS - Continued
Flexibility: Ability to kneel on the floor, bend at the waist and knees, go from standing to the floor while assisting an individual who may be falling, or get into any position that may be required to provide individual related care during emergencies.
Standing: Ability to stand/work on feet for 25% or more of the scheduled shift.
Sitting: Ability to sit periodically throughout the shift to complete required documentation; Sitting may be required for at least 25% of the scheduled shift.
Experience: One year experience working with people with developmental disabilities required.
Technology: Knowledge of Microsoft applications. Ability and willingness to learn computer applications as they may become required for documentation. Computation skills required.
Communication: Demonstrated ability to communicate verbally and to interact professionally. Ability to perform written documentation clearly, accurately and completely. Demonstrated ability to organize, prioritize, and complete tasks independently required.
Certifications: Direct Service Provider certification or the ability to obtain within 120 days of employment.
