Please note that individual total compensation for this position will be determined at the Company's sole discretion and the wage range for this role considers a wide range of factors including but not limited to skill sets; experience and training; licensure and certifications; and other business and organizational needs. At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $50,572.14-$68,421.12


Position Summary:

This QA/QIC II position functions within and across Sacramento Native American Health Center (SNAHC) by supporting the SNAHC’s Continuous Quality Improvement (CQI) Program as described in the policies and procedures as well as the annual plan. The QA/QI program staff are both technical assistance and leaders in internal agency improvement. Scope of responsibility encompasses all settings of the organization. Additionally, this Coach is intended to provide progressive support for clinicians through external changes, such as annual coding updates or documentation requirements responsive to Value-Based Care models.

The Quality Improvement facilitator(s) will provide the leadership necessary to achieve national best practice performance levels in quality and patient safety while guiding staff on how to implement and monitor evidence-based quality and process improvement practices.

The QA/QIC II works directly with agency staff supporting their work on intra-departmental goals for improvement. As all SNAHC staff are accountable for CQI goals and objectives the coaches are extra-departmental. Performance at this level requires knowledge of the science of improvement, communication and facilitation, and a familiarity with primary ambulatory healthcare. The overall impact desired through this implementation aligns with the Institute for Healthcare Improvement’s Triple Aim (improving care, improving population outcomes, and reducing cost per-capita.)

Essential Functions:

  • Work with the Compliance Officer to introduce and implement components of annual QA/QI Plan.
  • Support individual department staff as they conduct and document quality improvement processes and activities that align with organizational goals.
  • Analyzes patient, staff and stakeholder survey data and identifies plans for quality improvement as well as review patient experience and incident reports, patient complaints and grievances and report on trends.
  • Facilitate Quality Improvement Committee. Develop meeting agendas and facilitate meetings, and advise and assure that quality improvement ideas are carried out and evaluated.
  • Develops, assists with, and monitors systems, and procedures for the identification, collection, and analysis of the performance of quality measurement data.
  • Train new and existing employees and Board Members during orientation and as needed thereafter.
  • Communicate data verbally and through Excel, PowerPoint presentations and trainings.
  • Track improvement activities documented and reported within the organization to verify staff are engaged in CQI.
  • Work with the Compliance staff to help prepare departments for impending and periodic audits, both internal and external.
  • Ensure processes, documentation, and measurement are compliant with required standards based on executed contracts and provider manuals.
  • Review improvement reports and provide feedback to staff in a timely and educational manner which results in documentation that meets Accreditation Association for Ambulatory Health Care standards (i.e., the AAAHC Study format.)
  • Recommend changes for quality improvement in organizational policies, procedures and protocols based on findings and information.
  • Maintain professional standards and practices in all activities and services.
  • Assess quality improvement training needs; plan and implement training for agency staff per request of departmental leaders.
  • Execute training for improvement and feedback for the program.
  • Other duties as assigned.

Minimum Qualifications:

  • One year experience working with Quality Improvement Toolkit (i.e., Ishikawa diagrams, PDSA, root-cause analysis, process-mapping);
  • Ability to coach staff and promote spread of evidence-based techniques in performance-improvement and communication such as AAAHC, TeamStepps, Patient-Centered Health Home, LEAN, etc. ;
  • Ability to assist staff in creating SMART goals for measurable improvement ;
  • Functional understanding of changing Center for Medicaid and Medicare Services (CMS) regulations and requirements ;
  • Knowledge of Geographic Managed Care standards and benchmarks (HEDIS, CAHPS, etc.);
  • Ability to coach care team members on proposed Value-Based Care implementations (i.e., Medicare Hierarchical Condition Category Coding or “HCC”, Risk Adjustment Factors or “RAF”) ;
  • Ability to communicate clearly, both verbally and in writing, with a diverse group of staff, both internal and external to the organization;
  • Excellent training, facilitation, and interpersonal skills;
  • Business writing and analytical reasoning, ability to interpret and evaluate complex information;
  • Functional knowledge of Microsoft Suite of Applications (Excel, Word, Power BI, Teams, Outlook, SharePoint.)
