The Program Supervisor of Resident Wellness provides training and assistance to residents of the supportive housing program in order to help them attain and maintain optimal physical health by coordinating health care planning with residents’ health care practitioners. The PS will supervise the specialty positions of Dietician, Health Care Facilitator, Occupational Therapist, and Addiction Specialist. The Program Supervisor is responsible for sites located in Nassau and Suffolk County and will be required to work in both our Hempstead and Ronkonkoma offices.
- Coordinate and be a liaison with medical providers for all Supportive Housing residents.
- Problem-solve to manage acute and chronic medical conditions.
- Liaise with home health aides as needed.
- Arrange for group visits by certain providers who do house visits (podiatrists, dentists, etc.).
- Receive and organize all health reports, medical incident reports, and other related documentation.
- Facilitate and ensure follow-up medical recommendations are implemented.
- Coordinate referrals to Home Health Care agencies.
- Work with program staff to develop strategies to meet any special needs that a participant may have.
- Provide resources for obtaining affordable durable medical equipment.
- Collect data on medically-oriented outcomes.
- Identify health-related training needs for staff and residents.
- Assist with referrals to medical day programs.
- Maintain directory of facilities and establish relationships at such facilities in event of client discharge or acute stay.
- In collaboration with other team members, direct the establishment of short and long term goals related to management of health care.
- Assess need for and implement special accommodations in accordance with Options’ Accessibility Plan.
- Write progress notes.
- Supervise Wellness Team activities. Evaluate the performance of team members; provide regular feedback and coaching regarding employee performance. Address disciplinary issues as needed. Meet with staff regularly, but at least monthly.
- Responsible for appropriately staffing the team including making hiring and termination decisions.
- Provide supervision and guidance and/or case specific supervision to staff to assist in their professional development, client interactions and goal development.
- Review and assign incoming referrals to corresponding specialty positions according to client needs.
Schedule:
Monday - Friday: 8:00 AM - 4:00 PM (35 hours/week)

- Must be a Licensed Practical Nurse (LPN); RN a plus.
- Bachelor’s degree in human service-related discipline, or in a mental health discipline, and one year of experience working with persons with psychiatric disabilities.
- Supervisory experience preferred.
- At least 5 years of qualifying experience may be substituted for Bachelor’s degree.
- A valid driver’s license and safe driving record are required.
- QUALIFYING EXPERIENCE: Verifiable full-time residential or case management experience with persons with a history of mental illness.
