Population Health Coordinator

Under the general supervision of the Director of Primary Care (DPC) the Population Health Coordinator (PHC) is responsible for assisting with population health projects, provider schedule management, and monthly quality reports. The PHC will step into positions to ensure the department needs are met and acts as a liaison between clinic administration, clinic nursing, informatics and quality department to ensure maximum collaboration.

Essential Functions:
Assists the DPC in running and processing monthly patient quality gap reports. Responsible for reporting out data to appropriate leaders. Reports population health measures to Accountable Care Organizations, Care Coordination Organizations and other insurance carriers which offer quality incentives.

• Answers phone calls with proper etiquette and can accurately obtain and document patient information. Consistently exhibits behavior and communication skills that demonstrate Curry Health Network's commitment to superior customer service, including quality, care and concern with each internal and external consumer.
• Schedules appointments appropriately based on the providers' templates. Works with the DPC to develop scripting for patient outreach. Acts as a liaison between the clinical staff to determine provider preferences.
• Responsible for assisting with population health projects including running reports, scrubbing charts, and contacting patients for scheduling. Responsible for tracking patient appointments and gathering data for reporting to quality.
• Practices and adheres to the “Code of Conduct” philosophy and performs other duties as assigned to support the overall effectiveness in accordance with CHN’s Mission, Vision and Shared Values.

Minimum Education & Experience:
High school graduate or equivalent.

• At least one year of experience in patient access or population health is required. Two years of experience is preferred.
• Experience running reports from CPSI is preferred.
• Experience with data entry is preferred.

Required Knowledge, Skills & Abilities:
Knowledge of medical terminology and procedures.

• Knowledge of insurance plans and referral procedures.
• Knowledge of office practices and procedures.
• Ability to operate various office equipment including computer and various software programs.
• Ability to maintain confidentiality.
