Our Physical Therapist oversee all (home based) therapy care based off the plan of care for patients located within an Arlington, Crystal City, Old Town, Falls Church North, Tysons, territory.

You will complete and submit Oasis Assessments, Transfers, Resumption of Care, Discharges and Significant change in condition in accordance with Agency time frames.

The Physical Therapist will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide skilled nursing care to patients on an intermittent basis.

Required Qualifications:

• Graduate of an approved school of Physical Therapy
• Current and valid Virginia PT License
• Passionate and motivated to provide care within a patient's home.
• Ability to exercise initiative and independent judgment.
• Comfortable working collaboratively in cross-functional teams
• Ability to communicate effectively, both verbally and in writing, with all levels of an organization
• Ability and willingness to travel within a given territory.
• Willingness to work with multiple patients in varying home locations.

Preferred Qualifications:

• Two (2) years' home health nursing experience, preferred.
• Previous experience with Home Care Home Base EMR system strongly preferred.
