You Will Be Responsible For:

Provides support to direct patient care programs
- Provides accurate, safe, timely, and appropriate medication therapy based on patient needs
- Places practice priority on the delivery of patient-centered care
- Identifies high-risk patients and implements interventions to improve quality and safety
- Makes appropriate evidence-based, patient-centered medication recommendations
- Provides effective medication education and counseling to patients and caregivers
- Proficiently maintains patient information and efficiently evaluates patients’ health care needs
What You Will Need:
Required:
- One year of Contemporary hospital pharmacy experience
- Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree
- Applicable State Pharmacy License
Job Summary:
Prepares and organizes pharmacy medications by name and dosage, as listed on patient orders.
