Responsible for development and implementation of programs and materials that serve to promote education on HIV risk prevention and treatment.

Essential Functions:

· Facilitates enrollment in HIV Benefit and Entitlement programs.

· Conducts adherence assessments and evaluations with HIV positive patients; i.e.: reducing barriers to care.

· Works cooperatively and collaboratively with entire HIV Medial and Behavioral Health Provider Team.

· Provides community outreach and community education programs to targeted community agencies.

· Maintains data and case records as required.

· Provides appropriate follow-up on referred patients.

· Participates in case conferences as well as other agency or community based meetings or trainings.

· Refers those in need of support services and/or other community sources.

· Develops linkages and refers patients for additional service supports.

· Conducts HIV counseling and testing.

· Ability to travel to Sun River Health locations as needed

EDUCATION/EXPERIENCE:

High School Diploma/GED required with 1-2 years relevant experience

Peer Certification preferred or willing to complete during employment required
