Provides orientation of the Ryan White funded HIV care and supportive services program and prevention education from the main office or other MMG locations; completes program related screenings; acts as the primary liaison between the patients, the Primary Care Physician, various providers, referral sources and external service providers. Verifies program and insurance eligibility, collects required information, increases likelihood of retention in care with the scheduling of appointments, providing reminder calls, appointment letters and outreach as needed. Documents all engagement in the EMR and data bases as required by grant/program funding; navigates patients through the health system and related processes; provides insurance navigation as needed (ADAP, referrals to Medicaid Interviewers, etc).

Provides navigation services to patients to increase the likelihood of retention in care by scheduling appts, providing reminder calls for appts, mailing appointment letters and outreach as needed; provides insurance navigation for under or uninsured participants , and linking them with payment support programs and/or referrals to internal Medicaid Interviews or for ADAP assistance.

Provides education on the benefits of PrEP for partners, DOHMH's partner services program; navigates patients through the health system and related processes.

Knowledge of the HIV epidemic in Bronx and NY, HIV prevention, PrEP and PEP guidelines (preferred not required)

Required:

• Associate's Degree Required/Bachelor's Degree Preferred
• 5 years of work experience required
• Knowledge of health and social issues affecting LGBTQ population in the Bronx
• Experience with ambulatory medial facilities, health centers/ FQHC's &/or health systems
• Knowledge of LGBTQ serving agencies in the Bronx and NYC
• Community-based or other experience working with HIV positive individuals, mentally ill and chemically addicted (or both) individuals,homeless or transient individuals and other special populations
• Bilingual-Spanish
• Multi-tasking is a required skill and may present as challenging in high volume community-based, in-office locations and/or at health centers.
