JOB SUMMARY

The Patient Care Navigator is an experienced member of the clinical team whose role is to ensure high quality of overall care for our patients. This position is primarily responsible for outreach to patients who need preventive health services. Strong Excel skills are needed to manage the outreach activities driven by electronic data reports and requests from clinical teams. Other duties include helping patients with patient assistance programs; providing patient information about SCHC, other agency and community resources; and considering/completing advance care planning. If the Patient Care Navigator is a trained clinical staff member they may serve as a backup for the regular clinical support staff with tasks such as rooming patients, administering injections, or completing prior authorizations, but this should be a minimal amount of time.


JOB DUTIES AND RESPONSIBILITIES

  • Outreach to patients due for preventive health services or chronic disease management services.
  • Develop and maintain an open and regular line of communication between clinical teams, patients and outside agencies.
  • Coordinate with other professionals who help patients, such as social workers, hospitals, and other local agencies.
  • Connect patients to community resources as it relates to housing, food, transportation etc.
  • Perform general case management and mitigate patient barriers to care and refer patients to appropriate services as needed.
  • Conduct data entry and ensure consistent and accurate data is captured in patient charts during chart digging process.
  • Utilize different data platforms to perform appropriate outreach for QI department and track outreach efforts.
  • Participate or assist QI coordinators in yearly QI campaigns.
  • Utilize resources and coordinate care while assisting patients to achieve preventive care services or chronic disease management goals.
  • Develops and conducts outreach with patients via phone/text/mail for preventive health or chronic disease management services.
  • Assists in maintaining monthly dashboards displaying performance on different Quality metrics for specific SCHC site, provider, department etc.
  • Attends and facilitates (as assigned) quality topics at different all staff meetings, department meetings and QI committee meetings.
  • Provide patient education during outreach calls as it relates to quality improvement activities (which might include but are not limited to):
    • Encourage compliance with preventative and chronic disease management services.
    • Provides reminders to patients on upcoming preventive health screening appointments
    • Educate patients on the importance of routine health visits
  • Promote SCHC programs and services to the community as needs arise.
  • Enhance professional growth and development through participation in education programs, workshops and in-service meetings.
  • Self-directed and able to organize and manage multiple tasks.
  • Perform other duties as assigned.

KNOWLEDGE, SKILLS, AND ABILITIES

  • Moderate to advanced skills in a Microsoft Windows environment, including use of Microsoft Office suite (Outlook, Word, Excel, PowerPoint).
  • Electronic health records experience is highly preferable (NextGen experience a plus).
  • Proficiency in verbal and written communication in English.
  • Ability to read, write, understand and spell English and medical terminology correctly.
  • Demonstrated ability to work effectively in a team environment.
  • Moderate to advanced problem-solving skills.
  • Sound judgment to support making appropriate decisions to benefit patients and meet SCHC objectives.
  • Ability to prioritize tasks and complete on a timely basis with minimal supervision.
  • Ability to deal with change and seek out opportunities to effect change to promote patient care.
  • Strong customer service approach to problem solving situations.

EDUCATION & EXPERIENCE

  • Two years of experience as Medical Assistant or three years of experience in patient education, medical or health-care workplace, case management, or similar experience required.
  • High school diploma or equivalent.
  • Type 45 WPM or greater.
  • Excellent people skills and customer service orientation.


PREFERRED

  • 2 years of college education and experience in healthcare field
  • Experience manipulating and analyzing data.
  • Experience with electronic health records.
  • Certified Medical Assistant.
  • Experience working with patients to connect to health and community resources.
  • Basic knowledge of medical terminology.
  • Bilingual, English/Spanish.
