PACE Partners of Northeast Florida, also known as PACE (Program for All-Inclusive Care for the Elderly), is a joint venture between Community Hospice & Palliative Care and Aging True. Care coordination for the program, the first of its kind in North Florida, is delivered at our beautiful day center known as The PACE Place.

Our clinic and adult day center is located on the Westside of Jacksonville, next door to the Acosta-Rua Center for Caring. Job Summary:
Supervise: This position is an individual contributor role.
The Dietary Aide serves meals to participants and assists in various processes related to the preparation and delivery of snacks and meals to the participants. This includes learning and implementing texture modifications and adaptive medical diets such as diets for diabetes, cardiovascular disease, and chronic kidney disease. The Dietary Aide will also set up trays, utensils, and adaptive equipment according to participants needs. Dietary aide will help with assisted feedings as needed and documentation of meal consumption of participants. The Dietary Aide will also complete general cleaning, documentation, and sanitation of work areas to adhere to SERV Safe kitchen standards. The Dietary Aide will be able to assist in nutrition presentations to participants as time allows.
Qualifications and Requirements:
Education/Training/Certifications:
  • High School diploma or GED, required
  • SERV Safe food handler’s certification, or equivalent
  • Education on adaptive diet textures preferred
  • Reliable transportation
Experience:
  • 6-months or more, food service experience
  • Experience working in geriatric clinical food service preferred
Medical Clearance
Employees must have medical clearance for communicable diseases and have up-to-date immunizations before direct participant contact.

  • Work Environment: Employee must be able to work effectively within an interdisciplinary team model, interfacing and collaborating with a wide range of clinical and social services disciplines who work together to manage the PACE Partners participants' care. The work setting is in an Adult Day Health Center and primary care clinic environment with moderate noise levels and controlled temperatures.
  • Physical Requirements - Requires manual and finger dexterity, hand-eye coordination, the ability to use department equipment and the ability to lift/carry up to 30 pounds using appropriate body mechanics with reasonable accommodation if needed.
  • Visual, Hearing and Communication Requirements - Requires corrected vision and hearing to normal range, with or without reasonable accommodation. Ability to communicate by way of the telephone with participants, customers, vendors, and staff.
  • Pressure Factor - Requires working under stressful conditions. Working conditions may be noisy and crowded, with fluctuating indoor temperatures. Moderate pressure to meet scheduled deadlines. Potential for exposure to verbal aggression by participants, vendors, and or employees.
  • Environmental Conditions - May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases, and any other conditions common in a healthcare environment. Subject to unpleasant odors.
Duties and Responsibilities:
  • Assists in preparation of meals and snacks while adapting to the textural and medical diet accommodations of each participant
  • Sets up trays and utensils for food service
  • Completes cleaning and sanitation of the kitchen equipment
  • Washes dishes and tables
  • Sweeping and moping of floors
  • Maintains kitchen safety standards through required documentation including but not limited to taking refrigerator and freezer temperatures and practicing safe labeling and storage of food items
  • Practices safe food handling in accordance with PACE kitchen policies and procedures and SERV Safe standards
  • Assists in other duties as assigned
