The Role: Outreach Case Manager
What You'll Do: You'll spend significant time in the field locating, engaging, and assessing the needs of those experiencing homelessness/sleeping in places not meant for human habitation. You'll build rapport and connect individuals and families to eligible services, coordinated entry systems, Veterans Administration (VA), and other mainstream resources throughout Richmond, Charlottesville and Petersburg that serve those with psychiatric disabilities, co-occurring or substance abuse disorders. Assist with service needs, including assistance gathering documentation for housing opportunities and advocating for clients to obtain permanent housing. Provide outreach services at designated walk-in centers, documenting case notes in our HMIS system. Act as an access and assessment point for veterans, screening potentially eligible individuals and families for participation in the SSVF program and other community services. Transport and accompany clients to appointments as needed.
What You'll Bring to VSH:
- A Bachelor's degree preferred and at least one year of direct service experience with individuals with low-incomes, disabilities, and/or histories of homelessness.
- Knowledge and experience with services for Veterans strongly preferred.
- Knowledge of homelessness, mental illness, substance abuse disorders, community resources, and recovery concepts.
- Skill in establishing supportive trusting relationships with persons experiencing homelessness and with professionals in the homeless services and Veterans services systems.
- Ability to approach homeless individuals and build rapport through empathy, concern and understanding.
- Willingness to work in all-weather on foot or by vehicle, plus a valid driver license and ability to travel within Richmond, Charlottesville and Petersburg.
