SUMMARY

Looking for a motivated, self driven Occupational Therapist to provide Occupational Therapy Services to the participants of our Adult Day Health center. This is a great opportunity for someone looking for flexibility in their schedule to have a career, but still maintain a comfortable home/family/social balance. The Occupational Therapist will be resonsible for providing skilled occupational therapy services consistent with professional standards of practice to participants attending our Adult Day Health Center. EDUCATION

Minimum of a bachelor’s degree from an accredited Occupational Therapy program.

EXPERIENCE

Prefer a minimum of one year of experience in a therapist capacity.

Preferred experience in home health, skilled nursing facility and/or geriatric/adult outpatient therapy services.

QUALIFICATIONS

Licensed as an Occupational Therapist in the state(s) of practice.

REQUIREMENTS

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS

• Must hold the knowledge of basic computer literacy
• Provides occupational therapy in accordance with physician's orders and accepted professional standards of practice.
• Performs patient centered evaluations to determine functional level and establish a therapeutic plan of care.
• Develops an overall therapy plan of care to include treatment and medical diagnosis warranting treatment , the planned duration and frequency of therapy to be provided, a minimum of long-term goals and discharge plans; per standards of practice.
• Utilizes standardized assessments upon the development of the plan of care to validate the need for skilled therapy services and demonstrate repetition of this standardized assessment to reflect the ongoing need for skilled therapy and the patient 's progress.
• Completes all documentation required and in accordance with state and federal regulations.
• Documents observations, patient 's response to treatment, treatment programs provided, degree of progress and notable changes in patient's condition.
• Completes all billing required on the date of the provided treatment, and in accordance with all state and federal regulations, and company policy.
• Communicates and coordinates patient's care with other members of the patient' s health care team and the patient.
• Supervises occupational therapy assistants in accordance with regulations.

Job Description: Occupational Therapist

• Provides patient, family and caregiver teaching related to the patient's occupational therapy program, safety, and functional status.
• Assist the patient and/or family to procure needed equipment or supplies for optimal functioning and safety both in and out of the home environment.
• Utilize appropriate infection control practices including adherence of PPE, hand washing, standard precautions and personal protective equipment per state and federal regulations, and company policy.
• Hold the skills of and demonstrate strong time management and organization skills, as well as seamless communication.
• Maintain a positive attitude within compliance to the company' s code of conduct.
• Participates in scheduled trainings and/or educational sessions.
• Provides proof of attendance at mandatory in-services or training programs per company policy.
• Maintain skills and education by attending continuing education programs.
• Responsible for meeting designated productivity expectations as directed by the Director of Rehabilitation.
• Other duties as required.
