About The Role:

We are seeking experienced and driven Nurse Practitioners to provide care at our 3 clinics in the greater NYC area.

You will serve as a key resource for patients, should have strong clinical skills and should be well organized, friendly, a team player, pay attention to detail, and be comfortable working independently under the direction of our Medical Director.

Key Responsibilities:

Deliver primary care services to patients on-site at one of our three Clinics under the guidance of the Medical Director including but not limited to the following:

• Perform physical exams and patient observations; Diagnose health issues
• Record patients' medical histories and symptoms
• Create patient care plans and contribute to/modify existing treatment plans as necessary
• Order, administer, and analyze diagnostic tests
• Monitor and operate medical equipment, as needed
• Administer medicine and other treatments
• Consult with healthcare professionals and families
• Train patients and their families to manage and prevent illness and injury
• May assist with training efforts of new APPs and provide clinical direction to lower credentialed clinical team members under the guidance of the Medical Director
• Ensure patient care and safety are a consistent company priority and work to remain compliant with all applicable regulatory requirements
• Collaborate with Medical Director and leadership to identify areas of opportunity, optimize policies and procedures, drive growth, and disseminate information to clinic staff
• Participates in the monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement
• Participate as necessary with clinical quality audit and peer review processes per established practice protocols

Requirements:

• Current, unrestricted, New York State (NYS) RN and NP licenses/registrations
• Master's degree in nursing, prescriptive privileges (or eligibility)
• 3+ years of clinical experience in family or internal medicine required; pediatric experience a plus

• Must be able to positively interact with other clinicians, senior management, patients and their families, and all levels of medical and non-medical professionals
• High sense of urgency and can-do attitude required
• Technical acumen with EMR system ([Athena] experience a plus)
• Self-motivated and willing to own assigned responsibilities with minimal supervision
