Qualifications:
- Registered Nurse with a current New York State license.
- Nursing degree from an accredited college or university. Graduation from an approved school of Nursing and experience may be considered in lieu of a degree.
- Previous experience in a long-term care setting, SNF/nursing home highly preferred.
- Knowledge of MDS, Care Plans and DOH regulations for LTC facilities.
- IV certification a plus.
Responsibilities:
Knowledge of regulatory requirements governing long term care facilities.
Participates in the development, interpretation and implementation of the nursing department’s philosophy, objectives, standards, policies and procedures.
Supervises and directs the activities of all nursing personnel on their unit to assure the provision of quality care in a safe and therapeutic environment. Makes rounds at least twice per shift to each unit assure same and assess and residents, staff, or other pertinent issues.
May direct other team members as warranted.
Responsible for the evaluation and monitoring of all levels of resident care through on-site observations and audits, including the monitoring and the evaluation of Care Plans for quality, appropriateness and effectiveness on their unit.
Responsible for coordination of the activities/resident care on the evening and night shift for their unit.
Oversee/coordinate the tasks of the Night Supervisors for their unit.
Assures through supervision, monitoring and auditing that all responsibilities of staff job descriptions are fulfilled.
Completes staff performance evaluations as necessary.
Initiates, coordinates and participates in all investigations and evaluations of accidents/incidents involving their unit.
Emphasizes through on-site monitoring and direction adherence to residents’ rights in all aspects of care.
May be responsible to carry out Admissions Nurse job responsibilities.
Ensures that documentation on 24 hour report is accurate/ thorough and follows through on all residents listed on the 24 hour report.
Monitors all residents on oxygen, tube feedings, suctioning, pressure ulcer care protocols, and behavioral problems, and any other residents’ needs.
Emphasizes through direction, monitoring and auditing the importance and requirements of Infection Control in all aspects of care.
Ensures that nursing staffing patterns and assignments scheduled are maintained.
Conducts regular meetings with staff.
Each unit manager is responsible to review the daily schedule.
Participation in QI program through completion of audits and developing/implementing corrective action.
Responsible to identify educational needs.
Inservice staff in general staff meetings, specific unit meetings and individual meetings as necessary.
Identify staff issues; ensure compliance to standards, policies and philosophies. Institute disciplinary measures as appropriate.
Communicates with DNS/ADNS on disciplinary issues to be followed through and make Nursing Administration aware of staff issues.
Attend seminars, conferences, etc., as deemed necessary.
Ensure through observation, documentation and audits that all residents with pressure ulcers have documentation following the specific pressure ulcers protocol.
Ensures any orders written by the physician have been noted, picked up and transcribed correctly and reviewed.
Serves as a role model in professionalism, conduct, appearance and continuous improvement.
Maintains open communication with the DNS/ADNS/RN Manager on all issues relevant to residents and staff.
Fosters an atmosphere for continuous improvement and growth in the long term care field.
Responsible for coordinating and assisting in all resident and staff emergencies; assures that emergency procedures are followed through; reports to DNS/ADNS/Designee/Administrator On Call, where appropriate.
Maintains the confidentiality of all resident information as per HIPAA guidelines.
Serves as a role model in professionalism, conduct, appearance and continuous improvement.
Keeps Administration/Nursing Administration advised of potential/actual safety hazards.
Identifies educational needs of Nursing staff and provides individual, group or unit education/in-service.
