Essential Functions

  • The Case Manager role involves the use of critical thinking skills and clinical expertise to provide ongoing case and care management to an assigned panel of patients.
  • This will include: Managing the ongoing care management for an assigned population case load (Population Health Management),
  • Performing Comprehensive Risk Assessments and developing individualized Patient Care Plans for all assigned cases,
  • Managing and coordinating patient access to care, including overseeing the efforts of collaborating Care Coordinators, Referral staff and others,
  • Addressing all relevant Transitions of Care management when patients obtain hospital-based care, and
  • Collaborative participation in the Patient Care Teams.
  • Performs various population-health activities such as patient calls, mailings, assembles patient information packets, etc., including conducting pre-visit preparations for a select group of patients.
  • Accepts clinical responsibility for patient caseload and to organize it effectively and efficiently regarding clinical priorities and use of time.
  • Manages the delivery of care to an active caseload of patients by self and other agencies to reduce the risk of ED utilization, hospital admissions and of deteriorating health - using a patient-centered care plan with their caregiver(s) and the interdisciplinary team.
  • Tracks and monitors care utilization for a select group of patients.
  • Participates in the disease management with emphasis on patient education and self-care


EXPANDED FUNCTIONS

Complex Population Care Management

Applies nursing theory and research evidence-based practices for the care management of complex patient populations, such as:

  • Children under the age of 18 years of age with complex special needs
  • Transplant or ESRD patients
  • Individuals with an Inpatient stay greater than 21 days
  • Individuals discharged from an acute rehabilitation or skilled nursing facility
  • Individuals taking multiple medications with complicated treatment plans


Education, Experience and Skills Required

  • Associate degree in Nursing from an accredited college or university is minimally required
  • Bachelor’s degree in Nursing from an accredited college or university is required to facilitate complex disease care management and advanced patient needs
  • Registered Nurse (R.N.) with a valid license in the state of Illinois
  • Current CPR Certification
  • a minimum of two (2) years experience as a qualified Case Manager preferred.
  • Experience of working in a community-based environment.
  • Experience of working with highly complex packages of care in a health and/or social care environment.
  • Bilingual ability preferred; may or may not be required based on individual department needs.
