     Job Summary
    
 
    
     The navigator role will support the Gill Structural Heart program. The program requires a high level of communication with patients and referring providers pre and post clinic, pre and post-surgery, ongoing evaluation and monitoring of surgical, implantable, and medical therapies. The navigator will be instrumental is patient coordination for valve conference, retrieval of medical records, scheduling, verifications of authorizations, and communication to patients and their families. This will also include support data management STS/ACC TVT Registry and future ACC Transcatheter Valve Certification.
     
 
     
This position has been identified as supporting UK HealthCare, which receives funding from the Centers for Medicare and Medicaid Services (CMS). Therefore, all individuals hired into this position must comply with the federal CMS mandate issued in March of 2022. This mandate requires all new hires to provide proof of COVID-19 vaccination or obtain an approved medical or religious exemption as a condition of employment. Start of employment cannot occur until two weeks after receiving the full COVID-19 vaccination series or upon obtaining an approved exemption. Only vaccines approved by the World Health Organization can be accepted.

     Use of electronic medical record, good problem-solving skills. Excellent communication and customer service skills. Knowledge of and experience working with community-based resources providing services to the identified patient population.
