SUMMARY:
CCS is a voluntary psychosocial rehabilitative Medicaid program for eligible residents (children and adults) of Milwaukee County. CCS focuses on helping people who have a mental health and/or a substance use diagnosis on their journey to recovery. Those who qualify for CCS work with Care Coordinators to design a recovery plan of their choice. CCS offers a variety of unique rehabilitative services, including peer support, supported employment, nutritionists, and wellness activities such as yoga and meditation. More traditional services such as psychiatry, psychotherapy and medication management are also available.

DUTIES AND RESPONSIBILITIES:
  • Work with the CCS Administrator to maintain overall clinical oversight of the program
  • Monitor and ensure that CCS meets agency, state and federal standards including DHS 36
  • Work with program staff to determine consumers need and eligibility for services.
  • Serve as the CCS team’s primary Mental Health Professional, providing a clinical level assessment of the services to assist in the development and implementation of comprehensive services to meet the mental health needs of the CCS consumers
  • Intervene and support staff in crisis and develop plans to address crisis as needed
  • Provide clinical supervision to include individual, group, team meetings and side by side direct supervision
  • Provide and facilitate services as needed including care coordination in the absence of designated staff
  • Design and implement appropriate systems changes
  • Participate and represent Broadstep in all meetings for the CCS Program within the agency and with Milwaukee County
  • And other duties as determined or assigned

SUPERVISORY RESPONSIBILITIES:
Directly supervises a team of care coordinators
QUALIFICATIONS:
Specific requirements include:
  • Minimum of a Master’s Degree in Social Work or a related field
  • LPC or LCSW (fully licensed, not In-Training)
  • Substance Abuse credentials preferred (but not necessary)
  • 3 years’ experience in the human services field with adults and youth, CCS experience preferred
  • Must have the ability to make accurate assessments, communicate effectively orally and in writing and interact effectively with consumers and other health care professionals.
