Position: Mental Health & Family Planning CHW
Reports to: Program Manager
Location: WellShare Headquarters, Minneapolis, MN
Classification: Non-exempt
Compensation: $18 – $22/per hour, generous benefits (3 weeks PTO, 401K match,
health, vision, dental insurance)

Using a community health worker (CHW) model, we work with underserved communities in Minnesota and rural Africa. Our programs serve people across the lifespan – including pregnant people, infants, children, youth, adults, and the elderly.
POSITION SUMMARY
WellShare Community Health Workers (CHW) are the heart of our team’s work to provide individual and group health education, assist people with health system navigation and social service referrals to support people’s ability to live the healthiest life possible. This may include home visits, community outreach, individual and group presentations, teaching ongoing classes and activities, building relationships with individuals and supporting partnerships with community agencies and health practitioners.
Work is conducted primarily during regular business hours, but some evening and weekend hours may be required. This position is based out of the WellShare International headquarters office in Minneapolis, with the potential for remote work as arranged by the program manager. Some projects include regular travel for client engagement around the Twin Cities.
PRIMARY RESPONSIBILITIES
Program implementation:
- Assist clients in identifying overall health/life needs and set goals in one on one, small group, and large group settings.
- Provide health education and coaching to clients on topics such as:
- stress reduction
- mental illness
- family planning
- sexually transmitted infections
- and healthy relationships
- Recruit, intake and maintain participants for programming
- Perform community outreach and engagement to develop partnerships and educate the community on related topics above.
- Recruit participants to attend community events and programming, leveraging partnerships and networking.
- Identify relevant and accessible community-based resources and provide referrals into services.
- Establish and maintain partnerships with community-based organizations and leaders.
Administration:
- Maintain accurate and organized program records in Word, Excel, and Google Calendar
- Provide clear and appropriate oral and written communication to community members and partners
- Work cooperatively with partners and stakeholders via email and in-person
- Attend partner meetings to represent WellShare and community needs
- Adhere to reporting requirements as outlined by Program Manager and/or Coordinator
- Electronic health record data-entry
Provide content and technical assistance:
- Contribute to the creation of education and outreach materials in print, video and social media.
Perform additional duties as assigned
JOB SPECIFICATIONS
- Willingness to consider pursuing CHW certification and/or CHW Certificate
- Passion for assisting individuals to meet their health goals
- Spanish language fluency
- Excellent oral and written skills in English
- 1 year of experience in health education and community outreach and engagement preferred
- Ability to use computers and related software including word processing, spreadsheets, basic database management, and internet-based tools
- Experience working with underserved populations
- Effective interpersonal communication skills
- Ability to work collaboratively with others in program planning and implementation
- Self-starter with ability to juggle multiple priorities and meet deadlines
TOOLS AND EQUIPMENT USED
- Incumbent must be able to use multi-line telephone, calculator, copy machines, computers, and other office equipment.
- Incumbent must be able to travel to multiple locations within a day with a personal vehicle, possess a valid driver’s license and up-to-date insurance coverage, in order to drive to meetings, activities, and events throughout the State of Minnesota.
WORK ENVIRONMENT
- Incumbent primarily works in an indoor office setting. Incumbent must be able to go outside in all MN weather conditions and varying temperatures to attend off-site meetings and events.
