Job Title: MDS Assessor
Location: Fairview Skilled Rehab Nursing Home

Job Summary:
As an MDS Assessor, you will be responsible for assessing patients in the nursing home and documenting their medical history, physical condition, and functional abilities. You will work with the interdisciplinary team to develop individualized care plans and ensure that residents receive appropriate care and services. The ideal candidate will have excellent communication skills and be able to work effectively with a diverse population.
Key Responsibilities:
- Assess residents using the Minimum Data Set (MDS) tool.
- Develop individualized care plans for each resident based on their assessment.
- Monitor and evaluate residents' health and functional status.
- Participate in interdisciplinary team meetings to discuss residents' care plans and progress.
- Collaborate with physicians, nurses, and other healthcare professionals to provide appropriate care and services.
- Ensure that all documentation is complete and accurate.
- Adhere to all state and federal regulations.
Qualifications:
- Bachelor's degree in nursing or a related field.
- Minimum of 2 years of experience in a long-term care setting.
- Experience with MDS assessments and care planning.
- Knowledge of state and federal regulations related to long-term care.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
