MA Care Coordinator Job at PedIM Healthcare
Currently looking for an exceptional Care Coordinator with medical experience to join our team. The right applicant will be completing monthly health assessments while focusing on patient's chronic conditions via telephone (non face-to-face). This is someone who should possess excellent customer service, written and verbal communication skills. The candidate must be able to build a good rapport with patients. Other skills necessary to complete day to day duties are: working and documenting within an EMR (electronic medical record) system, problem solving, reviewing charts, orders, labs, consults, requesting records from specialists, reviewing orders, labs, consults, send medication refills, send referrals, and schedule appointments. The care coordinator will be an advocate for the patient and work closely with patients care team. The care coordinator will also be offering education regarding chronic conditions and encouraging good health habits.
Requirements:Working and documenting within an EMR (electronic medical record) system
Problem solving
Reviewing charts, orders, labs, consult notes
Requesting records from specialists
Reviewing orders, labs, consults, send medication refills, send referrals, and scheduling appointments.
