Job Title: LPN

Lifespire is currently seeking Licensed Practical Nurses to work hands on with individuals with developmental disabilities.

Position Summary: Under the supervision of a registered nurse, provide nursing services to consumers, coordinate program participants' services with hospitals, outside providers and the Agency. In conjunction with the QMRP, ensure that all medical recommendations are carried out and brought to a resolution. Participate in the interdisciplinary team, maintain appropriate medical records.

Job Duties and Responsibilities include the following. Other duties may be assigned:

Function A: Under the supervision of a registered nurse, evaluate consumers' medical needs and health status on a continuous basis, develop medical histories, nursing care assessments, nursing care plans, implement appropriate protective and preventive measures in regards to communicable diseases and maintain all appropriate medical records

Function B: Ensure consumers' prescriptions are filled/refilled, physician's medication orders recorded, monitor consumers' medication administration sheets to assure that medications are being administered properly, report medication errors appropriately, dispose of outdated or discontinued drugs according to policy.

Function C: Continually monitor the effects of consumer's medications for potential adverse reactions, allergies, interactions and contraindications.

Function D: Prepare, schedule and provide continuing in-service training for residential staff in all required and needed subject areas, train consumers in appropriate health and hygiene methods.

Function E: Comply with all agency standards, rules, practices, policies and procedures.

Education and/or Experience: Must be licensed by the New York State Department of Clinical Licensure, specialized training or one year experience preferably working with the developmentally disabled.

Physical Demands: Requires moderate physical effort on a regular basis, (See applicable checklist)

Working Conditions: Apartment setting or group home environment where consumers receive structure of daily routines through supervision. Occasionally stressful environment due to challenging behavior on the part of some consumers, therefore, must be able to maintain calm in the face of verbal, physical and emotional challenges from consumers. Exposure to individuals who may be aggressive and may carry communicable diseases.

Job Types: Full-time, Part-time

Pay: From $25.00 per hour

• Staten Island, NY: Reliably commute or planning to relocate before starting work (Required)

• BLS Certification (Preferred)
• LPN (Preferred)
