Provides care to individuals within established guidelines in the clinic and community settings.
Provides Client Services for the WIC Program. Reviews health history, anthropometrics, immunization screening, hemoglobin or hematocrit testing, evaluation of risk status, prescription of WIC food package, documentation, and high-risk referrals.
Provides and documents individual nutrition education counseling and follow-up for participants with low-risk medical conditions. Plans, conducts, and evaluates groups nutrition education classes.
Provides Client Services for the Immunization Program. Reviews immunization status, administers age appropriate vaccines per standing orders, completes SC Certificate of Immunization. Orders, stores, and processes vaccine in accordance with DHEC policy and procedures.
Administers medication, documents care, and works in conjunction with the registered nurse to monitor complex clients. Provides follow-up care in the home or clinic setting under the supervision of a registered nurse.
State Minimum Requirements: Positions in this classification require licensure as a Licensed Practical Nurse by the South Carolina Board of Nursing. Graduation from an accredited school of practical nursing and related experience.
Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.
DHEC employees are required to participate in disaster preparedness during hazardous weather or declaration of an emergency. This involves being subject to duty and or call on a 24-hour basis.
You must possess a valid driver's license and have access to a reliable transportation.
This position may require working early (before 8:30 am), evening (after 5:00 pm) and/or Saturday's.
Ability to lift 30 pounds and stand for long periods of time, if required.
