CCHC New Bern Internal Medicine Specialists is seeking a Wellness Nurse to join our team and provide outpatient care to our patients. The successful candidate will have experience in providing health education, conducting medical screenings, and developing individualized care plans. The ideal candidate will have excellent communication and interpersonal skills, be able to work independently, and have a strong commitment to patient wellness.
Responsibilities:
- Monitor patient's health and provide medical advice.
- Provide patient education on health and wellness topics.
- Conduct physical assessments of patients.
- Provide patient care in an outpatient setting.
- Maintain accurate and up-to-date medical records.
- Assist in the development of patient care plans.
- Collaborate with other healthcare professionals to coordinate patient care.
- Allscripts experience helpful
- Full-time, Monday-Friday
